Section 1
Why Kerala is Different
Three things make Kerala counselling unique among all Indian states.
28+ Community Categories
Kerala has the most granular community-based reservation system in India. Instead of OBC/SC/ST, there are 28+ community codes: SM (State Merit), EZ (Ezhava), MU (Muslim), LA (Latin Catholic), DV (Dheevara), VK (Vishwakarma), BH (Hindu Backward), and many more. Each is a separate competition pool.
Phases, Not Rounds
Kerala runs counselling in 3 Phases through CEE Kerala. Phase 1 is initial allotment. Phase 2 and Phase 3 redistribute vacated seats. You must confirm options online before each phase or lose your higher-order options permanently (Clause 11.6.4).
Single Window System (SWS)
All MBBS/BDS counselling — govt seats, private GQ, MQ, NRI, and minority quota — happens through CEE's Centralised Allotment Process (CAP) via a single online portal. You register options (course-college combos) in priority order. Trial allotments run before Phase 1.
CEE Kerala (Commissioner of Entrance Examinations) conducts counselling
The Commissioner of Entrance Examinations (CEE Kerala) conducts all MBBS counselling including government seats, private Government Quota, Management Quota, NRI seats, and Minority Quota seats in self-financing colleges (as per Supreme Court order dated 09.05.2017 on WP(C) No. 267/2017). The 15% All India Merit seats in self-financing colleges are also allotted through CEE (not MCC). Deemed universities go through MCC. Source: KEAM 2026 Prospectus, Clauses 1.7 and 2.2
Section 2
Eligibility & Nativity
Kerala classifies candidates into three nativity categories. Your category determines which seats you can access. Source: Prospectus Clause 6.1
Keralite
A candidate of Kerala origin — born in Kerala, or parent born in Kerala. Proof: SSLC showing Kerala birth, or parent's SSLC, or passport, or birth certificate, or Village Officer certificate.
Children of All India Service officers allotted to Kerala cadre are deemed Keralite but cannot claim community reservation or fee concession.
Access: All govt seats, all private GQ, community reservation, fee concessions, special quotas.
Non-Keralite Category I (NK-I)
Not of Kerala origin but meets any one:
(a) Studied qualifying course in Kerala + parent in GoI/Defence posted to Kerala
(b) Studied in Kerala + parent served Kerala govt for 2+ years
(c) Resident of Kerala for 5 years within 12 years of study
(d) School studies in Kerala from Std VIII to XII
Access: State Merit (SM) seats only. No community reservation, no fee concession.
Non-Keralite Category II (NK-II)
Everyone else. Not eligible for MBBS in government colleges.
Access: Only 15% All India Merit seats in self-financing MBBS colleges (allotted by CEE based on NEET rank, irrespective of domicile). No community/special/PwD reservation. No fee concession.
Also eligible for MCC AIQ (15% govt seats) and MCC Deemed counselling.
Academic eligibility for MBBS/BDS: Must have passed HSE (or equivalent) with Physics, Chemistry, Biology/Biotechnology and English individually, with minimum 50% in PCB combined. Relaxed to 40% for SC/ST, SEBC, and PwD candidates. Age: must complete 17 years by 31 Dec 2026. No relaxation in minimum age. Upper age limit as per NEET UG 2026 Information Bulletin. No rounding of percentage (e.g. 50/100 = 50%, not 49.5 rounded up). Must qualify NEET UG 2026 as per Clause 9.7.5(ii) & (iii). (Clauses 6.2.1, 6.2.5, 6.2.8, 6.3)
OCI/PIO candidates treated at par with Indian citizens for admission but cannot claim reservation or fee concession (Supreme Court judgement WP(c) 891/2021).

28+ categories = 28+ separate competitions
Kerala has the most complex reservation system for MBBS in India. An Ezhava (EZ) student at NEET rank 5,000 and a Muslim (MU) student at rank 5,000 have entirely different outcomes. Each community code has its own seat allocation, its own closing rank, and its own competition dynamics.
SEBC candidates must have Non-Creamy Layer certificate from Village Officer (for state education purpose specifically — central govt or employment certificates are NOT accepted). OEC candidates can claim un-availed SC/ST seats.
Palakkad Exception
Govt Medical College, Palakkad (IIMS) has a special reservation: SC 70%, ST 2%, SM 13%, AIQ 15%. Radically different from all other colleges. (GO(Ms) No.46/2015/SCSTDD)
Kollam Exception
Govt Medical College, Kollam: State Quota 50%, ESIC 35% (not via CEE), AIQ 15%. The 35% ESIC seats are for wards of insured persons. (GO(Rt.)No.1621/2017/H&FWD)
Seat Conversion Rules
Unavailed SC seats go to ST and vice versa. Remaining go to OEC, then SM. Unavailed SEBC and EWS seats go to SM. NRI/Minority conversion in self-financing: NRI-Minority → NRI-Open → SM; Minority → SM. (Clause 5.5)
PwD Reservation
5% supernumerary in all govt/aided colleges for candidates with 40%+ benchmark disability. Medical Board verifies suitability. Must qualify NEET UG. (Clause 5.3)
SEBC reservation applies only to Keralite candidates with Non-Creamy Layer status. NK-I and NK-II cannot claim community reservation.

Management Quota
Management seats in self-financing colleges. Also allotted through CEE's CAP. Higher fees than GQ.
Eligibility: NEET qualified. Keralite and NK-I eligible. Fee as fixed by FRC/government.
NRI
NRI Quota
NRI seats at self-financing colleges. Also through CEE's CAP. NRI sponsor = father/mother/siblings (incl. first cousins)/spouse/siblings of parents (incl. first cousins)/half-siblings/adopted parents working abroad. (GO(MS) No.243/2014/H&FWD)
Eligibility: NRI sponsor documentation per GO(Rt)375/2020, GO(Rt)1480/2022, GO(Rt)1616/2024 & GO(Rt)1556/2024. Highest fee tier. Clause 5.7 & 7.6.
15% All India Merit in Self-Financing
15% of seats in self-financing MBBS colleges are for All India Merit — allotted by CEE (not MCC) based on NEET rank, irrespective of domicile. This is how NK-II candidates get into Kerala private colleges. No communal/special/PwD reservation or fee concession for these seats. (Clause 4.1.1(ii))
Minority Quota
Self-financing colleges with Christian or Muslim minority status have minority community quota seats. Filled through CEE's CAP. Candidate must belong to that minority community. Need Community/Minority certificate from Village Officer. (Clause 5.6)

Government (1,855 seats): GMC Thiruvananthapuram (250), GMC Kozhikode (250), GMC Kottayam (175), GMC Alappuzha (175), GMC Thrissur (175), GMC Manjeri (110), GMC Ernakulam (110), GMC Parippally/Kollam (110), GMC Palakkad (100), GMC Pariyaram/Kannur (100), GMC Idukki (100), GMC Konni/Pathanamthitta (100), GMC Kasaragod (50), GMC Wayanad (50). Private Self-Financing (3,400 seats): 21 colleges from Al-Azhar (250) to Kerala Medical College (150). Final seat counts published before CAP 2026. Self-financing seats split into GQ, MQ, NRI, Minority, and 15% All India Merit — all allotted through CEE. (Clauses 1.7, 2.5.2)

Who gets fee exemption?
Full fee exemption: SC/ST candidates (merit or reserved seats), OEC candidates (govt/community quota), children of inter-caste married couples where one parent is SC/ST, inmates of Sri Chitra Home/Nirbhaya Home/Juvenile Homes.
Fee concession: SEBC communities in Annexure X(a) with annual family income up to ₹6 lakh. Children of registered fishermen. Income-based scholarships for others.
No fee concession: Minority/NRI/Management quota admissions. NK-I and NK-II candidates regardless of income.

Govt MBBS at ₹5K–15K/yr = under ₹70K total for 4.5 years. Self-financing GQ at ₹5L/yr = ₹22.5L total. Self-financing MQ at ₹15L/yr = ₹67.5L total.
Fee Payment Rules
Fee shown in Allotment Memo must be paid within stipulated time. If you move to a higher-fee course in next phase, pay the difference. If lower-fee, excess refunded after admissions close. No interest on refunds. (Clause 12.2)

Non-Domicile Access
Not from Kerala? Here are your actual options, verified from the 2026 Prospectus.
NK-II = No govt college access
Non-Keralite Category II (NK-II) candidates are not eligible for admission to MBBS in government colleges through state counselling. Your OBC/SC/ST from another state does not work in Kerala. NK-II candidates cannot claim community, special, or PwD reservation, and get no fee concession. (Clause 6.1(iii))
15% All India Merit (Self-Financing)
15% of self-financing MBBS seats are allotted by CEE irrespective of domicile. This is the primary NK-II route into Kerala self-financing colleges. Apply through KEAM 2026 online. (Clause 4.1.1(ii))
MCC AIQ (15% Govt)
15% of government college seats go to MCC. Open to all India with standard reservation. This is the only non-domicile route to Kerala government colleges. Separate MCC process.
NRI Quota (Self-Financing)
NRI seats via CEE's CAP. Sponsor can be father/mother/siblings (including first cousins)/husband/wife/siblings (incl. first cousins) of father or mother/half-brother/half-sister/adopted father or mother — working abroad. Unallotted NRI-Minority seats convert to NRI-Open, then to SM. (Clauses 5.7 & 5.5.10)
Deemed — MCC
1 deemed university via MCC deemed counselling. Open to all India. No domicile restriction.
NK-I candidates (5yr residency / Std VIII-XII in Kerala) can access SM seats but not community reservation. Kerala has very limited non-domicile access compared to states like Karnataka.

MBBS Liquidated Damages
If you quit after the third allotment in the same academic year — to join other courses/colleges or for any other purpose — you must pay ₹10,00,000 in liquidated damages. Applies to both govt and self-financing colleges allotted by CEE. Irrespective of income, nativity, or reservation status. Recoverable under Revenue Recovery Act.

Unnecessary Options Penalty
If you file/retain options but don't join on final-phase allotment, causing govt/management seats to lapse: ₹10L penalty for MBBS, ₹5L for BDS. Plus debarment from CEE for up to 2 years. Recoverable under Revenue Recovery Act. (Clause 12.2.4(c)(ii))

BDS Liquidated Damages

Debarment Risk
Quitting after 3rd allotment or causing seats to lapse can lead to debarment from CEE entrance exams and/or allotments for up to 2 years. This is in addition to the financial penalty.
Compulsory Rural Service
Students completing MBBS from government medical colleges in Kerala are liable to serve in rural areas after completion. Duration and terms as per government orders issued from time to time. This is a condition of admission. (Clause 13.1(c))
Option Registration Fee
Non-refundable if you don't join after allotment. Refunded only if you don't get any allotment. Adjusted to course fee if you join. CEE can fix higher registration fee for final rounds to prevent seat blocking. (Clause 11.4.8)
Discontinuation after 1st year: Liquidated damages as above OR remaining years' fees, whichever is higher. SC/ST/OEC candidates and those with family income below ₹75,000 are exempted from non-MBBS/BDS liquidated damages. Students joining NDA/Naval Academy/Defence institutions are exempted on compassionate grounds. (Clause 12.2.4(b))
No fee refund after last allotment. Candidates who cancel admission or don't take admission in the last phase are not eligible for fee refund. For cancellation before last allotment, refund rules apply. No processing fee can be levied by colleges when student moves to another college/course in subsequent CEE allotment. (Clauses 12.2.2, 12.2.3)
Bond must be executed in prescribed Annexure XXV format at admission. These rules apply to all categories including SC/ST for MBBS/BDS liquidated damages.

THE CAP SYSTEM — EVERY OPTION COUNTS
Kerala's option system means if you get allotted but don't join, you lose all options in that stream. Register only options you will actually join. If you retain higher options, you must click 'Confirm' before each phase or lose them permanently. Trial allotments before Phase 1 give an indicative picture — not a guarantee. Over 4.5 years, the GQ vs MQ difference at the same self-financing college can be ₹45L+.

Other Eligible Communities (OEC) — Annexure X
OEC candidates can claim un-availed SC/ST seats (after SC/ST inter-conversion). OEC with Non-Creamy Layer certificate get full fee exemption for govt/community quota. OEC without NCL status need Community Certificate from Village Officer for fee concession only. (Clauses 5.4.3(f), 12.1.1)
